What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will help relieve any unnecessary anxiety you may be feeling. We will have you fill out your complete medical and dental history and review it with you.
Please bring with you a list of prescription medications and contact information for your primary doctor and any specialists. Our office is in the process of going paperless. We will have you fill out your complete medical history and contact information in-office at your first appointment on an iPad. Please bring with you a list of prescription medications and contact information for your primary doctor and any specialists.
